When to pull out?
In the condom and remove yourself right after ejaculation. Always change the condom before having intercourse again. Pick up some spermicidal jelly for her too. Do not allow any semen to come in contact with her vaginal fluids.
Firstly, in addition to the condom she should have vaginal spermicide in her. The spermicidal lubed condoms do not have enough to make any difference. Have her get a tube of vaginal jelly with an applicator. This goes into her before you do!
With condoms you should withdraw as soon as your ejaculation is complete. Most condom "failures" are a result of sperm swimming around the open end.
By using spermicide and condom together you will achieve about the same degree of protection afforded by her being on the pill - way better than condoms alone.

Also, remember to hold the bottom of the condom when pulling out so that it doesn't slide off and anything "sneak" out.
Never open the small pack with the condom inside with your teeth. Don't rip it open with your nails. All in all, don't let anything sharp that could make a cut get near it (so be careful when you put it on).
Sorry, but I have to use my teeth to open the condom wrapper, when I open it. Just be very careful about it. AND she should be on her own birth control as well.
Yeah, I wish they would make condom wrappers easier to open. (Water based lubricants might be compatible with condoms, but not with wrappers and fingers!)
Now if only Liquid Latex would be good as a condom. Open the can and just dip! LOL

Otherwise, when you orgasm, the condom will blow up in the reservoir portion, and you're both in trouble.
I was joking, NewtoLove. Just picture possible "dipping" scenarios in your mind. All of them would be hilarious esp. since liquid latex needs some time to dry.
But it also does not stand up to fiction very well.
