If she is playing with her breasts and nipples then this activity is just part of self pleasuring or making out. Depending upon how a woman is "wired" breast and nipple stimulation can often augment the pleasure being derived from genital stimulation. For others, it becomes a distraction just prior and during an orgasm. It is rare although not unheard of that a person (female or male) can achieve a climax just from caressing their breasts.

Speaking as a woman, I think it has a lot to do with mood and anticipation with me. When he takes his time and slowly makes love to me with his tounge it's an incredible turn on.
