I'm a naturally very shy and sometimes awkward person which is why I am surprised that I only have one "worry" about sex for the first time...but this is what I am nervous about...what is a sexy way to put on a condom, or ask if he has one? Should I let him put it on, or should I put it on for him? Because in my mind safety is not always the first thing I think about when I get aroused and I don't want to ruin the mood or anything by making him stop for a second to roll a condom on. I guess my question is: How do I make sure he wears one without ruining the mood? Any suggestions would be appreciated...

Practice with a condom and a cucumber, English cucumber, carrot stick, etc. Learn how to roll one on and off.
As for who does it, either. If you apply it then you can keep him alert and at attention. It doesn't really matter who does it; however, I think it is great if the woman does it part of the time. Why? It shows her interest, doing so while continuing to stroke his penis will keep interest from waning from distraction.
I agree with the rule: NO CONDOM, NO SEX. As for the possibility of ruining the mood, this should not happen or be a problem if you integrate the roll on with the continuation of foreplay. The same goes for women who use a diaphragm. It is nice if the guy applies the spermicide and inserts the diaphragm. Just titillate the woman's pieces parts and have her do likewise.
I always prefer to put the condom on him. Makes that part of the sex play. When you are ready, tell him it is time for the condom, then roll it all the way against his body. Remember, vaginal jelly in you in addition to the condom makes things a lot safer.
For example, just earlier tonight:
"Do you have your items?" (meaning condoms)
"Yes, over there."
