I had a problem with one of my old partners that had a very small vagina. While I think my penis size is slightly below average regarding girth, we sometimes had problems having sex as it would hurt her (and sometimes even me!)
Although I am sure most guys would agree that small vagina is great, it's obviously not fun for her if she feels pain instead of pleasure.
I am conscious of the fact that high arousal helps, but that was not always the case. As far as I could tell, the most determining factors was the time of the month and pure unpredictable randomness (with a bias towards the former).
Some positions made it easier, but her departure has made impossible to learn enough to be of actual use to me.
So here is the question: What positions would you recommend for this situation? Any other tips not related to position?

Basically anything where she's not bent in half, as in doing that it's shortening up everything inside her.

In a word: Yes, although not because of the parameters you give. Just use pressure and do not poke, jab, or, shove. Even more important is to spend a minimum of half an hour, more within reason if possible, fooling around and making out. She requires this time to become fully aroused and you will benefit, also.
