How long for sperm to die?
At least half the time we do urinalysis for men, we see some sperm, living or dead. That is normal.
It causes great humour in the lab when we find sperm in a woman's sample!!
To answer your question sperm will live only a short time on its own simply because it needs a well regulated enviroment to survive. I doubt that your j/o in the morning before a physical is going to reveal any of your activities but do keep in mind the doctor will already know you masturbate so I wouldn't worry if I was you. It's a clinical fact that boys and girls masturbate alot, how you do it is up to you. Another fact is that boys also develop a slight callous on their penis from masturbation and this is more evident with circumcised boys as it also sometimes shows around the flare of the head and the area where the frenulum extends down from the head, guys without a frenulum build the callous in the 'V' area on the bottom side just below the head. Uncut guys show the callous around the front of the foreskin and depending on how they grab and hold the callous develops where the fingers normally hold.
If you're really scared the doctor is going to notice make sure you pee before leaving the house that way you can be sure it's all flushed out. Other than that and I hate to say it but you're fooling no one, boys that masturbate show it in ways you never thought were possible!

First of all, why would you worry about it?
Second, sperm do not live very long outside a VERY specific environment. Anything outside your body would definitely be dead by the time you reached the doctor and sat in the waiting room. (heck, sometimes I feel like I might grow old and die sitting in a doctor's waiting room...lol)
Chances are that unless you ejaculate just before you pee in the cup, there won't be any semen in your urine. And even if there were, the sperm would most certainly be dead from the acidity in the urine.
This really isn't something you need to worry about. And I really am kind of curious as to why you are. ??
