I have yet to hear of a coherent reason why couples tend to go with FMF's rather than the much easier to arrange MFM's. Given the female capacity for muliple orgasms and greater sexual stamina one would have supposed that MFM's would be preferred. Logisically if nothing else. Usually the woman comes up with something on the order of 'one dick is enough' - which isn't true, really - see dildo sales figures. So if it isn't the penis - it must be something else.

I think a lot of straight guys don't like the idea of sharing a bed with another guy. I know I wouldn't. But I'm very much a MF person anyway - ie: not into threesomes.
I'll add... that in the cases where there is bisexual interaction, it's more common for women to admit to those feelings than men. When a woman says she's bisexual most people seem to assume that she is "really" straight but just likes to play around. When a man says he's bisexual, the tendency of most people (gay and straight) is to say (or at least, think), "Oh, he's really gay but just can't admit it."
I personally believe that there are a lot more bisexual people out there than society usually credits... male, in particular. I think as society becomes more accepting of gay people then we'll see more bisexual men as well.

Someone also suggested that maybe it is the woman's ego that gets in the way - she wants to sideline him so she does girl on girl leaving him out of the action - but to me that would make an MFM even more attractive since hat would make her the natural center of attention!
