My wife's fantasy and mine both include another girl, but there is a fundamental difference than a normal threesome as the other girl would be there only to please us. (aka I wouldn't have sex with her, she would be there more for her tongue/hands for both of us.)
My instinct is to live out my fantasies (which is don't generally do), but I feel that hers is more of a mental turn-on? (Is that true for most women?) I do my best to please her, but the thought of sharing her with anyone isn't too high on my "things I want to do list." There is no easy answer because each couple needs to work this out for themselves. The only threesome I have ever had was with two other women, one bi and one a lesbian. We were all friends before and still are years later.
As a young woman, there were two couples of us who often travelled together. To save money we would get rooms with two double beds in cheap hotels. We always made for private time for each couple but there was still some action with four bare legs under sheetes after the light went out. Even then, there was no suggestion, or even thought on my part, of turning this into anything other than two couples doing their things with little privacy. I, personally, cannot imagine myself in a mixed gender threesome.
