Most women do not orgasm from penetration alone because most men have no real idea of what they are doing. There are two internal areas reached via penetration that will cause orgasms in women if they are properly stimulated and she is sufficiently aroused and relaxed. And she lets the orgasm come! You may simply require the services of a more highly skilled man.
The urge to urinate is the beginnings of an orgasm and you stopped it.
