How long can you have sex with a standard lubricated condom before microscopic holes begin to develop/ or until there is an increase of pregnancy?

About as long as anyone can last. Condom "failures" rarely involve holes; they are usually the result of sperm swimming around the open end. Withdrawal, holding the condom, should be immediately after ejaculation.
