Let us do some math. Assuming completely random, unprotected sex during a regular 28 day cycle and a 25% pregnancy success during ovulation gives us 5/28*1/4 or about a 4% chance of getting pregnant each time we have sex. This leads to a result that of 100 women having regular, random, unprotected sex for a year, 85 will become pregnant. Another way to say this is that if 100 women in your school have unprotected sex graduation night, four will become mothers. If anal sex, without condom, reduces this by a factor of ten, eight of those 100 would become pregnant from anal sex. That is as good as many contraceptives but is higher than we need to live with.

Sitting and telling patients, “you are pregnant,” a not uncommon response is, “I can’t be; he, er, put it in, er, the ‘other hole.’” The object of contraception is to reduce the chance of pregnancy. The intestinal lining in the rectum is one cell thick and is not absorbent. By the time the food gets that far through the digestive tract there is nothing worthwhile to absorb. Anal sex without a condom will result in leakage. Even the removal of the penis leaves a track of rich semen. With 300-400 million sperm in each ejaculation that leaves many to go seeking your vaginal entrance.

The majority of the semen deposited in the rectum, through the anus, drains out. It is draining about two centimeters from the vaginal entrance and has easy access through the folds. Sperm can, and do, easily make the swim.

It is also recommended, almost universally, that a condom always be used for anal sex. That helps. That would be kind of hard... since the reproductive organs in a girl go through the vagina. Her period comes out through there and the baby will too.
