Personally, I feel it depends on how close the guy is to orgasm. It's just like sex - at the start, short distances up and down (or in and out) might be okay, but when you're really close, you put it in as far as you can go.
Rhythm is also important. I think the guy can help you with this, and probably does if he's really into what you're doing. Actually, this works for girls too - if the one receiving the oral sex is liking it and is really into it, they start to move their hips back and forth in response to what you're doing. I think it's a pretty good indication they're not liking it if they're just laying there like a corpse.
To answer why we like blow jobs better than hand jobs, I think it might just be that A. we can do a better job ourselves (as smoove said) and B. that most of us have probably had more bad or painful hand jobs than blow jobs (I know I have).
