I've found that DT works best as an occasional part of the BJ, not the whole thing. There's that certain tantalizing moment for the guy of "okay, when's she going to do it?" followed by the "OMG, there it is!" That way, they don't get to the point where they EXPECT it -- it's more of a treat.
