First off, I just want to say that you are not in the small category. The average penis is between 4.5" and 5.5" and your circumference is also on par. I also want to say that the best lover I've had was your size, so it is much more how you use it than what size it is. The most sensitive nerve endings in the vagina are in the first 3 inches, so you have that area covered.
I would say that very few women would be turned off at your size. If your previous partner enjoyed it when you wore an extender, then good for you; you will do so if it pleases your next partner.
If you feel that you NEED to introduce it, I would be sure to talk about it before hand instead of just donning it mid-moment.
