I start with stroking by hand from outside the underwear. There's some good sensation to be had from the fabric to get things started. Then once they're off and I've done a little more gentle stroking by hand, I do some soft kissing up and down the vein on the underside, with just a little bit of suction from my lips on the skin. When I make it to the tip, before I take it in, I rub my lips around the corona a little too. Then once I've taken it in and I want to escalate things a little, I start pumping the shaft with one hand, and give a very gentle tickle with the fingers of other hand to the scrotum and around the base of the shaft. All three of those sensations all at once-mouth, pumping, and the gentle tickle-work really well.
