Its actually happened to me a few times, but usually only after a long period of abstinence. I think what happens is that I approach the point of orgasm, but because of the volume of semen in store it gets released before an orgasm happens. When this has happened, it has usually just flowed out in a steady stream. Trouble is, though, that seems to take away the need to orgasm and my erection will then usually subside (darn!). But not to worry, in that situation I've found that my recovery period is quite short, and I am usually able to go again fairly soon. Its a fairly rare occurrence though.
